The other scenario I thought about was the unique browser footprint. Info in server logs that is very specific to someone's machine and the way they have it setup (screen size, os, browser and a host of other info). In certain cases the probability becomes highly likely that it can be linked to another log where your identity is known because the same footprint (not sure if that's the correct word) is in that other log.
