Speaking from years of experience with a self hosted PHP application, I'd recommend a "recommend hosters" section in your FAQ. Setup an affiliate program with them - most of your users will be happy to send you the referral to know that you recommend and have a good experience with them.
Ones to really watch out for are Godaddy shared hosting (note that updates to .htaccess files can take many hours to actually show and performance is frequently abysmal) and "Hostgator" (they do something funky on some of their shared hosting servers that breaks custom user session support if you use that).
